Diabetes affects how the body controls blood glucose (or blood sugar). Diabetes can exist in two forms: as a chronic condition, type 1 and type 2 diabetes; or a reversible condition, gestational diabetes and pre-diabetes. According to the American Diabetes Association, diabetes affects more than 25 million people in the United States and is a leading cause of heart attack and stroke.
The physicians at Southside Physicians Network practices are dedicated to improving the quality of life for individuals living with diabetes through care and education. Our diabetes self-management program provides the skills and knowledge needed to manage the disease and avoid complications.
Our services include:
- Laboratory services on site at most primary care locations
- Point of service Hemoglobin A1C testing done during your visit
- Endocrinology services within the network
- Partnership with the YMCA to offer diabetic educational classes within your community.
