Requesting Records on Yourself

Print the Medical Records Request Form, fill it out completely and bring it in person or mail it to Release of Information at Grandview Medical Center along with a copy of your government issued photo ID.

Requesting Records on Others

Records may be requested by a parent or guardian of a minor child with the exception of psychiatric records. Records may be requested by persons holding an active Medical Power of Attorney. Call the Release of Information (ROI) Department to see if you qualify to request records on others. At that time the ROI Department will tell you what items to bring with you when you request records.
