Easy Street Inpatient Rehabilitation
At Grandview Medical Center's 17-bed rehabilitation center, Easy Street, you’ll work with a team of rehab specialists that includes the rehabilitation center’s medical director, occupational therapists, physical therapists, speech therapists and rehabilitation nurses. Your care team also includes a discharge planner to help you transition back to your normal routine, and a chaplain who provides both spiritual and emotional support during your journey.
For four consecutive years, the Centers for Medicare and Medicaid Services have ranked Easy Street in the Top 10% of inpatient rehab facilities in the United States.

What is acute inpatient rehab?
If you are experiencing limited function and are unable to care for yourself due to an illness or injury, you may benefit from rehabilitation. Patients often undergo inpatient rehabilitation if they are experiencing:
- Impaired mobility, balance and coordination
- Limited strength and range of motion
- Sensory loss or visual/perception problems
- Decreased ability to perform activities of daily living
- Swallowing disorders
Patients at Easy Street typically have a wide variety of diagnoses, including but not limited to:
A referral is needed to become an Easy Street patient. Referrals may be made for patients who are receiving care:
- In a hospital
- In a skilled nursing facility
- At a physician’s office visit
- At home
- At an outpatient rehab location
- At an assisted living or independent living facility
Patients can self-refer to the program, or any of the following can make a referral for you:
- Physicians
- Case managers and/or social workers
- Family
- Friends
- Allied health professionals such as physical and occupational therapists

Once a referral has been received, the coordinator will complete the paperwork assessing need for an inpatient rehab stay based on Medicare’s admission criteria. If appropriate, the coordinator will verify insurance requirements. Many insurance companies, especially HMOs, require pre-certification prior to admission to a rehab level of care. The coordinator will complete the process and will be in communication with the patient and family and/or caregiver every step of the way.
