At Northwest Health, we’re committed to getting you back in the game and we’re ready to help you Return to Play!
When it’s time for student athletes to return to play, we’ve got you covered. Simply visit one of the Northwest Physicians clinics listed here and follow the easy 3-step process:
1. The student athlete and a parent or guardian arrives at one of our participating clinics* with their school’s completed Return to Play form.
*Please note: Student athletes with ARKids First insurance who do not have a Northwest Health primary care provider must have a referral from their regular provider before they may be seen.
2. The clinic will register the student athlete as a new patient and a provider will see them and complete the provider’s portion of the paperwork.
3. The visit will be billed as a regular office visit to your insurance and any co-pay will be paid at this time.
